| We
provide hospice services to people who are ill with a disease
that medical science cannot cure. |
| |
  |
| |
|
...
not just for patients facing cancer. Hospice is
also for individuals facing lung disease, Alzheimer's,
AIDS, strokes, heart disease and other profound illnesses. |
| |
  |
| |
|
... not a place; it is a way of caring for people through
the in-home support of nurses, home health aides, social
workers, chaplains, counselors, volunteers and physicians. |
| |
  |
| |
|
as
much for the family as for the patient. Sojourn Care supports
the family and friends who are embracing the challenge
of caring for an individual with a profound illness at
home. |
|
| |
|
|
We take care
of people wherever they live: in their own homes; the homes
of friends; assisted living apartments or nursing homes. We
do not require our patients to have a full time caregiver
in the home. In fact, many live alone through choice or circumstance.
Based on patient
need, when specialized medical support is required, Sojourn
Care provides inpatient care or round-the-clock continuous
care where the patient lives.
We
make two simple promises:
|
 |
We will do
what we say we will do. |
 |
We will be
there when we say we will be there. |
