Patient/caregivers experiencing stress or difficulty because of the level of care required by the patient
Patient is not seeking curative treatment and is requesting comfort measures only
Hospice may be indicated for the following diseases with a life-limiting prognosis:
Assist physicians and healthcare professionals in determining
whether or not a patient is eligible for hospice care.
Can support the physician and other healthcare providers
with difficult conversations regarding prognosis.
Do not require a primary caregiver in the patient's home.
Will be swift in sending care into the patient's home.
Will collaborate with you on the Plan of Care.
Will provide oral or intravenous hydration, nutrition or
antibiotics consistent with the hospice Plan of Care.
Will support radiation, chemotherapy and advanced pain
management protocols consistent with the hospice Plan of Care.
