If any of these indicators are present, a hospice referral may be appropriate for your patient:
  Physical Indicators
    Life-limiting illness with a prognosis of 6 months or less (based on probability, not certainty)
    Declining functional status:
      1. Palliative Performance Scale score of less than 40%
2. Dependence in a majority of activities of daily living
    Frequent hospitalizations/trips to the ER/physician visits due to:
      1. Dehydration
2. Hypovolemia
3. Infections
4. Pain management
    Weight loss of 10% or more in the last 4-6 months
    Serum albumin level is 2.5 grams per deciliter or below
    Documented clinical disease progression with existing co-morbidities that limit the life expectancy
    Classified as New York Heart Association (NYHA) Class IV:
      1. Inability to carry out physical activity without discomfort
2. Symptoms of heart failure or angina at rest
3. Increased discomfort even with minimal exertion
    On the organ transplant list
     
  Other Indicators
    Patient/caregivers experiencing stress or difficulty because of the level of care required by the patient
    Patient is not seeking curative treatment and is requesting comfort measures only
     
Hospice may be indicated for the following diseases with a life-limiting prognosis:
    End-stage Cardiovascular Disease (CHF)
    Coronary Artery Disease
    End-stage Cerebrovascular Disease (CVA)
    End-stage Liver Disease
    End-stage Amyotrophic Lateral Sclerosis (ALS)
    Metastatic Malignancies/Cancer
    End-stage Neurological Diseases
    End-stage Pulmonary Disease (COPD)
    Adult Failure to Thrive
    End-stage Renal Disease
    End-stage AIDS/HIV
    Alzheimer's and Dementia
    Stroke and Coma
    General Referral Guidelines
The following tools are useful in assessing patient eligibility:
    New York Heart Association Functional Classification
    Body Mass Index
    Palliative Performance Scale
    Functional Assessment Staging (FAST)
Sojourn Care is easy to work with. We:
  Assist physicians and healthcare professionals in determining whether or not a patient is eligible for hospice care.
  Can support the physician and other healthcare providers with difficult conversations regarding prognosis.
  Do not require a primary caregiver in the patient's home.
  Will be swift in sending care into the patient's home.
  Will collaborate with you on the Plan of Care.
  Will provide oral or intravenous hydration, nutrition or antibiotics consistent with the hospice Plan of Care.
  Will support radiation, chemotherapy and advanced pain management protocols consistent with the hospice Plan of Care.
Based on your preferences, Sojourn Care will keep you informed of the care we provide to your patient.
