| Position: Social Worker |

| Summary of Responsibilities: |
| |
The Social Worker is responsible for providing medical social work services in accordance with the Plan of Care, under the direction of the attending physician and/or hospice medical director. |
| |
| Principal duties and accountabilities include, but are not limited to: |
| |
- Assessing the social and emotional factors related to the patient’s illness, his/her need for care, his/her response to treatment and adjustment to care and his/her financial status and need for resources.
- Participating as a member of the interdisciplinary team.
- With IDT and patient/family, developing a detailed Plan of Care.
- Assisting the IDT and other involved health care providers in understanding the significant social and emotional factors related to a patient’s hospice diagnosis.
- Assisting the patient/family with personal and environmental difficulties, accessing resources, such as family and community agencies.
- Completing all required documentation accurately and timely.
- Participating in on-call duties and admissions and deaths as necessary.
- Participating in in-service training for staff and contracted facilities.
- Complying with the state and federal regulatory requirements.
- Other duties as defined.

| Qualifications: |
| |
- A degree from a school accredited or approved by the Council on Social Work Education.
- Conforms to the requirements of the state licensure laws of social workers.
- Complies with acceptable professional standards and practice.
- Demonstrates good communication, negotiation, time management and public relations skills.
- Requires reliable transportation, current driver’s license and current auto insurance.
