1. Participating as a member of the interdisciplinary team.
2. Developing a detailed Plan of Care for each assigned patient in conjunction with IDT, patient and family.
3. Assuring all services indicated in the Plan of Care are provided, including coordinating services of other health care providers.
4. Reporting to the attending physician and hospice medical director any significant changes in the patient’s condition and requests change of orders as needed.
5. Informing patients and family about the services included in the plan of care, including their proposed frequency.
6. Informing the patients and families in advance of any changes in the plan of care.
7. Implementing medical treatment orders and patient follow up.
8. Teaching, supervising, and counseling patient/family regarding the nursing care needs, related concerns/issues of patient, services available and the Medicare Hospice Benefit.
9. Supervising the Licensed Practical Nurses and Home Health Aides/Certified Nurses Aides.
10. Tracking and recording Home Health Aide supervisory activities.
11. Completing all required documentation.
12. Participating in on-call duties and admissions as necessary.
13. Participating in in-service training for staff and contracted facilities.
14. Complying with the state and federal regulatory requirements.
15. Complying with agency formularies and protocol.
16. Other duties as defined.

1. Registered Nurse with current state license.
2. Minimum of one (1) year of recent medical, surgical, oncology or home care experience.
3. Complies with acceptable professional standards and practice.
4. Demonstrates good communication, negotiation, and public relations skills.
5. Requires reliable transportation, current driver’s license and current auto insurance.
