| Position: Licensed Practical Nurse |

| Summary of Responsibilities: |
| |
Under the supervision or direction of a registered nurse or physician the Licensed Practical Nurse provides nursing services to hospice patients. |
| |
| Principal duties and accountabilities include, but are not limited to: |
| |
- Participating as a member of the interdisciplinary team.
- Contributing to the assessment of the health status of patients.
- Participating in the development and modification of the Plan of Care.
- Providing safe and effective nursing care consistent with the Nursing Practice Act.
- Teaching basic nursing skills.
- Completing all required documentation.
- Participating in on-call duties and admissions as necessary.
- Participating in in-service training for staff and contracted facilities as assigned.
- Complying with the state and federal regulatory requirements.
- Complying with agency formularies and protocols.
- Other duties as defined.

| Qualifications: |
| |
- Licensed Practical Nurse with current state license.
- Minimum of one (1) year of recent medical, surgical, oncology or home care experience.
- Complies with acceptable professional standards and practice.
- Demonstrates good communication, negotiation, and public relations skills.
- Requires reliable transportation, current driver’s license and current auto insurance.
