| Position: Home Health Aide/Certified Nurse Assistant |

| Summary of Responsibilities: |
| |
The Home Health Aide/Certified Nurse Assistant is responsible for personal care services as determined by the Plan of Care, in accordance with a physician’s written orders and under the direction and supervision of a registered nurse. |
| |
| Principal duties and accountabilities include, but are not limited to: |
| |
- Delivering personal care services for each assigned patient per the Home Health Aide Assignment developed by the RN Case Manager.
- Assuring all services indicated in the Home Health Aide Assignment are provided, including personal care needs of patient, assistance with light duty household tasks, and/or assistance with exercise, ambulation or transfer needs.
- Participating in IDT as needed for quality patient care.
- Reporting to the RN Case Manager any appearance and/or behavioral changes in the patient’s condition and requests changes in Home Health Aide assignment as needed by patient/family.
- Completing all required documentation.
- Participating in after hours and weekend visits as necessary.
- Participating in in-service training for staff and contracted facilities.
- Complying with the state and federal regulatory requirements.
- Other duties as defined.

| Qualifications: |
| |
- Current Home Health Aide and/or Certified Nurses Assistant Certification.
- Prefer one (1) year of recent hospital or home care experience.
- Complies with acceptable professional standards and practice.
- Demonstrates good communication, negotiation, and public relations skills.
- Requires reliable transportation, current driver’s license and current auto insurance.
