| Position: Chaplain |

| Summary of Responsibilities: |
| |
The chaplain is responsible for the provision of spiritual counseling to support the patient and family, consistent with their belief system. |
| |
| Principal duties and accountabilities include, but are not limited to: |
| |
- Participating as a member of the interdisciplinary team.
- Assessing for spiritual needs and provide direct spiritual support and/or counseling to patients/families in keeping with their beliefs.
- Working with staff, clergy and community groups to enhance sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.
- With the IDT and patient/family, developing the Plan of Care.
- Providing bereavement follow-up services as defined in the bereavement Plan of Care.
- Completing all required documentation accurately and timely.
- Making contact with clergy or appropriate representatives of patients/families as indicated.
- Seeking the participation of volunteer clergy.
- Performing occasional liturgical assignments such as memorial services.
- Participating in on-call, deaths and admissions as necessary.
- Other duties as defined.

| Qualifications: |
| |
- An ordained pastor.
- Evidence of counseling education/experience.
- Demonstrated ability to be sensitive, flexible and ecumenical.
- Demonstrates good communication, negotiation, time management and public relation skills.
- Requires reliable transportation, current driver’s license and current auto insurance.
