  When science knows of no cure, Sojourn Care works to keep symptoms under control so that you can focus more clearly on what each day will bring.
     
  Symptoms aren't just physical; grief is real. Sojourn Care helps you deal with the feelings in your heart.
     
  You are eligible to receive hospice services when your physician believes that the illness could result in death in about six months. But, this prognosis is only a guess; no one really knows how long anyone will live. Once admitted to Sojourn Care, a patient can remain with us as long as the prognosis remains the same.
     
  Medicare and most other insurance plans will pay for hospice care. For Medicare patients, even the cost of related medications is covered. For patients in other insurance plans, out-of-pocket costs are low. Sojourn Care will work closely with your insurance company so you will know ahead of time exactly what your costs will be.
     
  We are available 24-hours a day seven-days a week to make visits, answer your questions or simply to reassure you in the middle of the night.
