are eligible to receive hospice services when your physician believes that
the illness could result in death in about six months. But,
this prognosis is only a guess; no one really knows how long
anyone will live. Once admitted to Sojourn Care, a patient can
remain with us as long as the prognosis remains the same. |

Medicare
and most other insurance plans will pay for hospice care. For Medicare patients, even the cost
of related medications is covered. For patients in other insurance
plans, out-of-pocket costs are low. Sojourn Care will work closely
with your insurance company so you will know ahead of time exactly
what your costs will be. |

are available 24-hours a day seven-days a week to make visits, answer your
questions or simply to reassure you in the middle of the night. |
